COA Outreach Services

Outreach:

The Outreach Worker makes home visits to assess needs and provide information regarding services and programs designed to maximize independence and support elder wellbeing. The Outreach Worker and Director respond to emergencies and provide on-going support to elders and families in need.

Information and Referral:

Along with information regarding transportation, housing and health information, the Outreach Worker provides local elders and their family members with information and referrals to other social service agencies.

Telephone Reassurance:

Providing home-bound elders with a friendly voice, the Outreach Worker makes reassurance telephone calls to local elders, assessing change and offering support as well as coordinating case management.

Meals on Wheels:

Available to eligible elders by calling AgeSpan at 1-800-892-0890.

Congregate Meals:

Located at 51 North Street, the COA offers nutritious Grab and Go lunches Monday-Thursday noon at the Georgetown Senior Community Center. Reservations must be made at least one week in advance by calling the COA or in person.

Public Health Nurse:

On the first Wednesday of each month, from 9:30 - 10:30 AM, Pam Lara, RN is available at the Georgetown Senior Community Center to perform Blood Pressure checks.
